Insurance Plans Accepted include but are not limited to: Medicare part B, Tradional Blue Cross Blue Shield, Medicaid (as secondary insurance), and Fidelis. If you have any other plan please verify coverage with your insurance company prior to visit. The patient is responsible for any deductibles, copayments, and non-covered services at the time of visit. Note: We do not accept Health Alliance Plan of Michigan (HAP) or United Healthcare at this time.
Cash visits are available upon request. Please download and return Cash Patient Agreement.

If you would like a New Patient Packet mailed to you, please call 248-607-7246. New Patient Packet's that lack required information will cause delays in scheduling an appointment.

New Patient Packets can be returned via fax, uploaded to the Patient Portal, or mailed back.
Patient Portal Upload Instructions:
- Scan New Patient Packet and safe as pdf.
- From Patient Dashboard Menu, Select "Additional Info Shared With Practice".
- Select "+Add Shared Information".
- In the field marked "Label" , type "New Patient Packet"
- Select Document
- Choose your scanned pdf file.
- Click "Submit"
